Family Connections, Inc. is a home and office based mental health agency located in Iowa and Nebraska.  Our offices in Iowa are located in Council Bluffs, Glenwood, West Des Moines, Woodbine, Avoca, Dunlap, and Sidney. Our offices in Nebraska are located in Bellevue. Family Connections, Inc. is dedicated to empowering, adults, children, adolescents, and their families to achieve self-sufficiency. We provide the full continuum of mental health services to aid families in successfully navigating life’s challenges.

Do you have trouble getting in to see your doctor for prescription refills, medication renewals, or routine medication reviews? Family Connections offers medication management services with our experienced Advanced Nurse Practitioners. We provide convenient appointments to help you stay on track with the medications you need. Telehealth Available
