Family Connections, Inc. is a home and office based mental health agency located in Iowa and Nebraska.  Our offices in Iowa are located in Council Bluffs, Glenwood, West Des Moines, Woodbine, Avoca, Dunlap, and Sidney. Our Nebraska location is located in Plattsmouth. Family Connections, Inc. is dedicated to empowering, adults, children, adolescents, and their families to achieve self-sufficiency. We provide the full continuum of mental health services to aid families in successfully navigating life’s challenges.

Do you have issues getting in to see your doctor for prescriptions or renewal/review of current medications? Family Connections is excited to announce the addition of our agency’s own Advanced Nurse Practitioners!!!!! They are now accepting appointments!!!! No more waiting months to get in to get your needed medications. If you have any questions or to schedule an appointment, please contact us at 712-256-4420 or 712-527-2823.

Telehealth Still Available

Family Connections, Inc. is dedicated to making sure that needed mental health services (therapy, in-home services and medication management) are made available to everyone so we will continue to offer telehealth to those who wish to utilize this service. We understand even though restrictions are lessening, some people may still feel comfortable with service delivery via telehealth.
